Objective: Several studies have provided prevalence estimates of posttraumatic stress disorder (PTSD) related to the September 11, 2001 (9/11) attacks in broadly affected populations, although without sufficiently addressing qualifying exposures required for assessing PTSD and estimating its prevalence. A premise that people throughout the New York City area were exposed to the attacks on the World Trade Center (WTC) towers and are thus at risk for developing PTSD has important implications for both prevalence estimates and service provision. This premise has not, however, been tested with respect to DSM-IV-TR criteria for PTSD. This study examined associations between geographic distance from the 9/11 attacks on the WTC and reported 9/11 trauma exposures, and the role of specific trauma exposures in the development of PTSD.
Methods: Approximately 3 years after the attacks, 379 surviving employees (102 with direct exposures, including 65 in the towers, and 277 with varied exposures) recruited from 8 affected organizations were interviewed using the Diagnostic Interview Schedule/Disaster Supplement and reassessed at 6 years. The estimated closest geographic distance from the WTC towers during the attacks and specific disaster exposures were compared with the development of 9/11–related PTSD as defined by the Diagnostic and Statistical Manual, Fourth Edition, Text Revision.
Results: The direct exposure zone was largely concentrated within a radius of 0.1 mi and completely contained within 0.75 mi of the towers. PTSD symptom criteria at any time after the disaster were met by 35% of people directly exposed to danger, 20% of those exposed only through witnessed experiences, and 35% of those exposed only through a close associate’s direct exposure. Outside these exposure groups, few possible sources of exposure were evident among the few who were symptomatic, most of whom had preexisting psychiatric illness.
Conclusions: Exposures deserve careful consideration among widely affected populations after large terrorist attacks when conducting clinical assessments, estimating the magnitude of population PTSD burdens, and projecting needs for specific mental health interventions.

Liver and muscle glycogen repletion using 13C magnetic resonance spectroscopy following ingestion of maltodextrin, galactose, protein and amino acids

The present study evaluated whether the inclusion of protein (PRO) and amino acids (AA) within a maltodextrin (MD) and galactose (GAL) recovery drink enhanced post-exercise liver and muscle glycogen repletion. A total of seven trained male cyclists completed two trials, separated by 7 d. Each trial involved 2 h of standardised intermittent cycling, followed by 4 h recovery. During recovery, one of two isoenergetic formulations, MD–GAL (0·9 g MD/kg body mass (BM) per h and 0·3 g GAL/kg BM per h) or MD–GAL-PRO+AA (0·5 g MD/kg BM per h, 0·3 g GAL/kg BM per h, 0·4 g whey PRO hydrolysate plus l-leucine and l-phenylalanine/kg BM per h) was ingested at every 30 min. Liver and muscle glycogen were measured after depletion exercise and at the end of recovery using 1H-13C-magnetic resonance spectroscopy. Despite higher postprandial insulin concentations for MD–GAL-PRO+AA compared with MD–GAL (61·3 (se 6·2) v. 29·6 (se 3·0) mU/l, (425·8 (se 43·1) v. 205·6 (se 20·8) pmol/l) P= 0·03), there were no significant differences in post-recovery liver (195·3 (se 2·6) v. 213·8 (se 18·0) mmol/l) or muscle glycogen concentrations (49·7 (se 4·0) v. 51·1 (se 7·9) mmol/l). The rate of muscle glycogen repletion was significantly higher for MD–GAL compared with MD–GAL-PRO+AA (5·8 (se 0·7) v. 3·7 (se 0·6) mmol/l per h, P= 0·04), while there were no significant differences in the rate of liver glycogen repletion (15·0 (se 2·5) v. 13·0 (se 2·7) mmol/l per h). PRO and AA within a MD–GAL recovery drink, compared with an isoenergetic mix of MD–GAL, did not enhance but matched liver and muscle glycogen recovery. This suggests that the increased postprandial insulinaemia only compensated for the lower MD content in the MD–GAL-PRO+AA treatment.

We model the dynamics of the human tear film during relaxation (after a blink) using lubrication theory and explore the effects of viscosity, surface tension, gravity and boundary conditions that specify the flux of tear fluid into or out of the domain. The governing nonlinear partial differential equation is solved on an overset grid by a method of lines using finite differences in space and an adaptive second-order backward difference formula solver in time. Our simulations in a two-dimensional domain are computed in the Overture computational framework. The flow around the boundary is sensitive to both our choice of flux boundary condition and the presence of gravity. The simulations recover features seen in one-dimensional simulations and capture some experimental observations of tear film dynamics around the lid margins. In some instances, the influx from the lacrimal gland splits with some fluid going along the upper lid towards the nasal canthus and some travelling around the temporal canthus and then along the lower lid. Tear supply can also push through some parts of the black line near the eyelid margins.

We consider model problems for the tear film over multiple blink cycles in limits that yield a single equation for the tear film; the single nonlinear partial differential equation that governs the film thickness arises from lubrication theory. The two models arise from considering the absence of naturally occurring surfactant and the case when the surfactant strongly affects the surface tension. The film is considered on a sinusoidally varying domain length with specified film thickness and volume flux at each end; only one end of the domain is moving, which is analogous to the upper eyelid moving with each blink. A main contribution of this article is computation of solutions for multiple complete blink cycles; the results of these non-trivial computations show a distinct similarity to quantitative in vivo observations of the tear film under partial blink conditions. A transition between periodic and non-periodic solutions has been estimated and this may be a criterion for what is effectively a full blink according to fluid dynamic considerations.

We describe a space VLBI experiment involving an earth-orbiting satellite (SURFSAT) and simulated satellites, a space VLBI ground tracking station, an array of ground radio telescopes, and a space VLBI correlator. The purpose of this experiment was to provide as complete as possible an end-to-end simulation of space VLBI in preparation for the first space VLBI mission, VSOP, and in particular to test the most critical aspect of space VLBI, viz. the ability to generate a stable and accurate frequency standard (clock) for the orbiting VLBI element.

Density functional theory provides a first-principles approach for computing the geometric and electronic structures, and a wealth of corresponding properties, of a wide range of materials types and compositions, including bulk solids, surfaces, defects and clusters of molecules. Parallel advances in hardware performance, implementation strategies and algorithms have all contributed to a rapid growth in the number of important applications. Recent developments under each of these themes are outlined and the breadth of current applications is illustrated by typical examples. Issues associated with the implementation and performance of density functional methods on parallel computer architectures are discussed.
